GUIDELINES ON FILING CLAIM

MOTOR CAR

    Own Damage
  1. Copy of LTO Certificate of Registration and Current Official Receipt;
  2. Copy of Policy and O.R. of Payment;
  3. Police Report / or Affidavit of Claim;
  4. Copy of Drivers License (Card and Official Receipt);
  5. Picture/s of Damaged Portion/s;
  6. Repair Estimate;
  7. In case of theft/carnapping loss :
    1. Alarm Sheet
    2. Complaint Sheet
    3. Certificate of Non-Recovery
    4. Set of Ignition Keys
    5. Original LTO Cert. of Registration and original current Official Receipt
  8. Upon payment of claim on Total/Theft Loss, an Absolute Deed of Sale shall be executed in favor of the insurer;
  9. If insured unit is mortgaged, Release of Chattel Mortgage must be submitted;
  10. Mortgaged Carnapped unit must contain Assured's and Mortgagor as payee; and
    Third Party Property Damage Claim
  1. Accident Report;
  2. Police Report /Affidavit of Claim (must establish liability of insured);
  3. Copy of LTO O.R. and C.R.;
  4. Copy of Drivers License and Official Receipt (OD & PD);
  5. Photograph/s of damaged part/s for both vehicles;
  6. Certificate of No-Claim by adverse insurance company;
  7. Estimate of repair/ repair invoice/ Official Receipt;
  8. Signed Affidavit of Desistance and Release of Claim.
    Third Party Bodily Injury / Death Claim
  1. Accident Report;
  2. Affidavit of Insured's Driver and Police Report;
  3. Copy of LTO Certificate of Registration and Official Receipt;
  4. Copy of Driver’s License and Official Receipt;
  5. Medical Certificate and other medical records;
  6. In Death Clam - Death Certificate and Certificate of Post Mortem Exam;
  7. Hospital / Medical Receipts, Invoice and other evidence of expenses;
  8. Funeral, Burial, Vigil Expenses;
  9. Birth Certificate, Marriage Contract, other proof of paternity/affiliation
  10. Signed Affidavit of Desistance / Release of Claim.

FIRE

    General Requirements
  1. Duly accomplished and Sworn Statement of Loss (Supplied by Adjusters);
  2. Duly accomplished and signed Non-Waiver Agreement (Supplied by adjuster);
  3. Affidavit of Loss stating the ff. info:
    1. Nature and Ownership of Property Insured;
    2. Location of Insured Property;
    3. Extent of damage of Property insured;
    4. Total amt. of claim;
    5. Liens and encumbrances, if any;
    6. Any info relative to the loss;
  4. Arson investigation report / fire investigation report / or portions of the property involved.
    Building
  1. Detailed repair / construction cost estimate of the building prepared by civil engineer and or architect;
  2. Building Plan / sketch of dimension;
  3. Building Permit or any document showing date of construction of building, repairs, renovation;
  4. Appraisal report, if any;
  5. Lease of Contract of agreement if the insured is not the lot owner;
  6. Tax Declaration of Real Property;
  7. Transfer Certificate of Title.
    Household Contents
  1. Detailed inventory of the affected and unaffected household contents with costing and date of acquisition;
  2. Proof of purchase or invoice, if any;
    Stocks
  1. Copy of the Mayor’s Permit and Business New Registration
  2. Copy of the Articles of Incorporation and By-Laws
  3. Copy of security and exchange commission (SEC) and Department of Trade and Industry (DTI)
  4. Income Tax return with Audited Financial Statement for the last 3 years
  5. Latest :Merchandise inventory filed with the BIR prior to the loss
  6. Copy of Purchase Invoices and Delivery Receipt from January up to the dated of the loss and
  7. Detailed Inventory of the affected and unaffected stocks ( finished and unfinished stocks including goods in process) showing its description, quantity, at cost unit price, extent of damage and amount of claim on each item.

PERSONAL ACCIDENT

    Death Claim
  1. Attending Physician's report;
  2. Police Report;
  3. Birth Certificate;
  4. Death Certificate;
  5. Autopsy report;
  6. Marriage Contract.
    Accident or Sickness Hospitalization or Dismemberment Claim
  1. Attending Physician's report;
  2. Police Report;
  3. Hospital Statement of account;
  4. Medical Bills and receipts (Original Copy).

MARINE CARGO

  1. Original Insurance Policy (in case of Marine Open Policy coverage, Insurance Certificate or Risk Note).
  2. Original Bill of Lading or Airway Bill of Lading with terms and conditions on reverse side.
  3. Original Commercial Invoice
  4. Original Packing List
  5. Duly received claim letter against :
  6. Waybill of Delivery Receipt (if already been delivered to intended receiver)
  7. Container Interchange Receipt or Container Interchange Report for Containerized Cargo
  8. Bad Order Certificate for cargo damaged during discharged & prior to delivery
  9. Copy of Entry filed with Bureau of Customs, if available
  10. Photographs, if any
  11. Consignee’s claim bill

SURETY BOND

  1. Notice of Claim
  2. Photocopy of Surety Bond
  3. Copy of formal complaint, if any
  4. Claim computation>
    1. statement of account
    2. breakdown of unremitted collections and penalty
    3. sample invoices, receipts and proofs of non-payment
  5. Demand Letter
  6. Response to the demand letter and payment proposal, if any
  7. Copy of Contract
  8. Application
    1. Name of company
    2. Type of service activity engaged in
    3. Name and address of guarantors
    4. Collaterals
    5. Other businesses to secure payment
