GUIDELINES ON FILING CLAIM
MOTOR CAR

Own Damage
- Copy of LTO Certificate of Registration and Current Official Receipt;
- Copy of Policy and O.R. of Payment;
- Police Report / or Affidavit of Claim;
- Copy of Drivers License (Card and Official Receipt);
- Picture/s of Damaged Portion/s;
- Repair Estimate;
- In case of theft/carnapping loss :
- Alarm Sheet
- Complaint Sheet
- Certificate of Non-Recovery
- Set of Ignition Keys
- Original LTO Cert. of Registration and original current Official Receipt
- Upon payment of claim on Total/Theft Loss, an Absolute Deed of Sale shall be executed in favor of the insurer;
- If insured unit is mortgaged, Release of Chattel Mortgage must be submitted;
- Mortgaged Carnapped unit must contain Assured's and Mortgagor as payee; and

Third Party Property Damage Claim
- Accident Report;
- Police Report /Affidavit of Claim (must establish liability of insured);
- Copy of LTO O.R. and C.R.;
- Copy of Drivers License and Official Receipt (OD & PD);
- Photograph/s of damaged part/s for both vehicles;
- Certificate of No-Claim by adverse insurance company;
- Estimate of repair/ repair invoice/ Official Receipt;
- Signed Affidavit of Desistance and Release of Claim.

Third Party Bodily Injury / Death Claim
- Accident Report;
- Affidavit of Insured's Driver and Police Report;
- Copy of LTO Certificate of Registration and Official Receipt;
- Copy of Driver’s License and Official Receipt;
- Medical Certificate and other medical records;
- In Death Clam - Death Certificate and Certificate of Post Mortem Exam;
- Hospital / Medical Receipts, Invoice and other evidence of expenses;
- Funeral, Burial, Vigil Expenses;
- Birth Certificate, Marriage Contract, other proof of paternity/affiliation
- Signed Affidavit of Desistance / Release of Claim.
FIRE

General Requirements
- Duly accomplished and Sworn Statement of Loss (Supplied by Adjusters);
- Duly accomplished and signed Non-Waiver Agreement (Supplied by adjuster);
- Affidavit of Loss stating the ff. info:
- Nature and Ownership of Property Insured;
- Location of Insured Property;
- Extent of damage of Property insured;
- Total amt. of claim;
- Liens and encumbrances, if any;
- Any info relative to the loss;
- Arson investigation report / fire investigation report / or portions of the property involved.

Building
- Detailed repair / construction cost estimate of the building prepared by civil engineer and or architect;
- Building Plan / sketch of dimension;
- Building Permit or any document showing date of construction of building, repairs, renovation;
- Appraisal report, if any;
- Lease of Contract of agreement if the insured is not the lot owner;
- Tax Declaration of Real Property;
- Transfer Certificate of Title.

Household Contents
- Detailed inventory of the affected and unaffected household contents with costing and date of acquisition;
- Proof of purchase or invoice, if any;

Stocks
- Copy of the Mayor’s Permit and Business New Registration
- Copy of the Articles of Incorporation and By-Laws
- Copy of security and exchange commission (SEC) and Department of Trade and Industry (DTI)
- Income Tax return with Audited Financial Statement for the last 3 years
- Latest :Merchandise inventory filed with the BIR prior to the loss
- Copy of Purchase Invoices and Delivery Receipt from January up to the dated of the loss and
- Detailed Inventory of the affected and unaffected stocks ( finished and unfinished stocks including goods in process) showing its description, quantity, at cost unit price, extent of damage and amount of claim on each item.
PERSONAL ACCIDENT

Death Claim
- Attending Physician's report;
- Police Report;
- Birth Certificate;
- Death Certificate;
- Autopsy report;
- Marriage Contract.

Accident or Sickness Hospitalization or Dismemberment Claim
- Attending Physician's report;

MARINE CARGO
- Original Insurance Policy (in case of Marine Open Policy coverage, Insurance Certificate or Risk Note).
- Original Bill of Lading or Airway Bill of Lading with terms and conditions on reverse side.
- Original Commercial Invoice
- Original Packing List
- Duly received claim letter against :
- Waybill of Delivery Receipt (if already been delivered to intended receiver)
- Container Interchange Receipt or Container Interchange Report for Containerized Cargo
- Bad Order Certificate for cargo damaged during discharged & prior to delivery
- Copy of Entry filed with Bureau of Customs, if available
- Photographs, if any
- Consignee’s claim bill
SURETY BOND
- Notice of Claim
